After you have made your appointment for your first visit we request that you complete our patient intake form online. For your convenience the new patient form is located in the Online Forms on this website. You can fill it out online, submit it and it will be incorporated into your patient record. This paperwork provides us with your health history and information on your condition. The more completely you answer the questions the better we can customize your care for a better result.
If you don't know if chiropractic is the right choice for you we offer a complimentary 15 minute consultation to discuss your health-related problems, concerns, and potential treatment options. This consultation is designed for you to ask questions and learn more about chiropractic care. In turn, we will ask you questions about your current health issues and past health history so we may determine if chiropractic care will be appropriate for you.
History and Examination
Once you have made the decision to try chiropractic care contact us and we will schedule your initial visit. At the time of your initial visit we will review your health history and your current health problems to learn more about you, your condition, and your expectations to determine how chiropractic care can meet your goals. Then Dr. Jay or Dr. Brenda will perform a complete chiropractic examination testing such things as your spinal function and motion. Other standard neurological, orthopedic, postural, and physical tests will be performed as well. However, nothing will be done in our office without your consent.
Depending on your specific condition, we may require that you obtain X-rays prior to receiving care. Current common protocol indicates that not every case requires this so not everyone will be required to obtain diagnostic imaging. However, X-rays can help us develop the most effective treatment plan for you, and alert us of any serious spinal conditions. Most people are amazed once they see their X-rays up close. In some cases, they can even identify their misalignments and degeneration themselves.
Report of Findings
Once the information is collected and an examination is performed, Dr. Jay or Dr. Brenda will give you a detailed report of all findings and answer any questions such as:
- How can you help me?
- How often do I need to come in?
- What will my treatment cost?
After reviewing your health history, goals, and examining your spine and X-rays, we will discuss recommendations and notify you if your condition requires care with other providers. Our goal is to provide you with the best treatment program for your specific needs.
At the conclusion of this initial appointment, if your chiropractor determines that chiropractic care is appropriate for your condition, you can choose to experience your first treatment. This may include spinal adjustments, joint mobilization and soft tissue massage. Treatment is interactive, so you can express concerns or ask questions about the different styles of treatment at any time during your visit. We encourage your feedbackand input as to how your body is responding to care. We want to know!
For most people the doctor may have suggestions for you to incorporate into your day. If you are in pain when you first come into our office this may include: ice or heat application instructions, certain activities or positions to avoid, and home exercises and/or stretches. Your active involvement with your care is crucial for faster recovery and improvement. If you desire, our healthcare team will work with you to create healthy habits and routines for your lifestyle. Every person is unique, therefore everyone requires a customized self care plan. The purpose of our self care program is for you to achieve good spinal alignment, have a healthy diet, exercise, and maintain a positive mental state.
